SPORT REGISTRATION INSTRUCTIONS
Hello Prospective Bomber Student-Athletes and Families!
We are very excited to continue our partnership with
FinalForms, an online forms and data management service. FinalForms allows you
to complete and sign required athletic participation forms for your
students. FinalForms saves data from season-to-season and year-to-year,
meaning that after you have registered, you will never need to enter the same
information twice, but merely update information as it changes.
FinalForms also pre-populates information wherever possible,
for each of your students, saving you time. You may review your data at any
time to verify it is current. You will be required to sign your forms once per
year and after any update.
You still are required to submit updated OHSAA physical
examination forms on a yearly basis to the Kenston Athletic Department. When
you first register with FinalForms, your last physical examination date will
not be in the system yet. If you have previously filed a form with the Athletic
Department, our staff has updated your account with that date information. If
you are unsure as to whether you have a valid form on file, please contact the
Athletic Department at your respective school.
In order to participate in tryouts or during the sport
season, your physical examination must have been administered within the last
13 months and be confirmed received by the Athletic Department – continuing
eligibility to participate in any sport expires upon the 13-month anniversary
of your last physical examination on file with the Athletic Department.
The preferred method to turn in physical forms is by
uploading scanned copies into your FinalForms account directly. Please DO NOT
hand these forms into a coach.
Registration with FinalForms is required for any athletic
participation (including tryouts). Please login and register immediately at
your earliest opportunity. Registration for all sports will open on May 1 of the academic year preceding the next fall sport season.
Please register now at FinalForms.com and follow the prompts
to create your account, register your students and electronically sign your
forms. Please note that both parent/guardian and students must log in to sign
the required forms.

support during the process, scroll to the page bottom and click “Use Support”.
